Acknowledgement Certificate
This form may be used by a notary public, and is usually attached to a document. Notaries are required to use the legal formats and verbiage of an acknowledgement certificate. When there is not a pre-printed section with this verbiage, this can simply be attached.

Credible Witness Affidavit
This form may be used when someone does not have a valid government-issued identification. A credible witness is placed under oath to then confirm the identity of a signer.

Limited POA for Vehicle Transactions
This form from the Texas DMV grants the grantee full power and authority to perform every act necessary and proper to purchase, transfer, and assign the legal title to the motor vehicle described on behalf of the grantor.

When a vehicle owner or the owner’s authorized representative presents this form filled out, presents payment, and presents a valid state or federally issued ID, the vehicle storage facility must release the vehicle.

Texas Notary Certificates
Texas notaries who are not licensed attorney's are provided with a menu of notarial certificates by the Texas Secretary of State. It is very important to note that a notary is prohibited from choosing the notarial certificate for your document(s), but a notary is allowed to provide you with the options available. You must choose which certificate is appropriate for your documents.

Sample Forms
