Acknowledgement Certificate
This form may be used by a notary public, and is usually attached to a document. Notaries are required to use the legal formats and verbiage of an acknowledgement certificate. When there is not a pre-printed section with this verbiage, this can simply be attached.
Credible Witness Affidavit
This form may be used when someone does not have a valid government-issued identification. A credible witness is placed under oath to then confirm the identity of a signer.
